USA Hockey Registration

To register, you will need a USA Hockey membershipt number.  CKHC is affiliated with MISSOURI hockey.  Do NOT select Illinois when applying for USA Hockey Membership.

Please email a cop of the player's birth certificate or passport to Registrar@collinsvillehockey.org

Registration Fees

Registration and Uniforms 

• Registration Fee (All Players) = $200

• Uniform Fee (All New Players) = $350

• Ice Fees (All Players)

  • 2025-2026 Season = $1250

SafeSport Training

The MVCHA requires its members including players 17 or older as of Dec 31st, coaches, officials and volunteers who have regular contact with or supervision over minor participants to complete the USA Hockey SafeSport training to ensure a safe environment for participants.  This training is available at no cost for registered USA Hockey individuals and is valid for one season, with a refresher course required annually to maintain certification after completing the core course.
